VA failed to process 56,000 requests to update veterans' dependents |

The VA said it failed to properly process 56,000 requests from veterans to add or remove dependents, some dating back as far as 2011. The mistake was discovered when VA staff looked into an error last month that caused problems for veterans filing PACT Act claims. Some veterans may be owed backdated benefits, while others may have been overpaid. Agency officials said they would not request reimbursement for those who received excess benefits. Affected veterans were told that the VA was conducting a review of the issue and they did not need to take any action, but were advised to pay attention to their mail for future VA requests for info. |

The VA may have mistakenly paid out more than $5 million in benefits to veterans claiming to have Gulf War Syndrome. Investigators estimate the VA "prematurely" approved around a quarter of the 13,800 Gulf War Syndrome claims filed between Oct. 1, 2021, and Mar. 31, 2022. Gulf War Syndrome encompasses multiple unexplained chronic afflictions found in tens of thousands of veterans who served in the Persian Gulf during Operations Desert Storm and Desert Shield. |
